Specimen Requirements


Please review the latest specimen requirements from Pathology Laboratory Associates (PLA) before sending samples for testing by clicking the button below.

Instructions for Requesting Tests

All orders should be placed with manual test requisitions. A list of requisition forms can be found in the Downloads Library.

For each patient, use a separate container (if multiple specimens and tests ordered) accompany with test request form. PLA may be unable to process specimens that are not in separate containers.

Use a black pen, as PLA will scan the request form for image retrieval.

Complete the information requested at the top of the form.

Mark an “x” in the box indicating the test(s) requested. Record the number of specimens submitted and total number of tests ordered on the test request form in the lower-left corner.

If there is no box for a test you are requesting, write the complete test name in the Other Tests Ordered box.

Keep a copy of the requisition for your record if needed. Please be sure the first page is sent to the laboratory with the sample.

The specimen container must have the patient’s first and last name and unique identifier (example DOB) and specimen type or source clearly indicated on it.

  • A cover letter summarizing the following:


    • Patient’s pertinent clinical and pathologic history, their diagnoses, and any specific questions to be addressed;

    • Contact information (name, phone, email) for the submitting clinical physician in case we need additional history;

    • Contact information for your laboratory, in case we need to request more material;


  • All pathology report(s); and

  • All slides for each specimen, including all levels, stained slides and controls from ancillary testing, and as appropriate, blocks or unstained slides.

  • Print a Pathology Consultation Requisition, and complete the form.

  • On the requisition form, choose the type of consultation requested and list the number of slides and blocks being submitted.

  • Provide the required documentation listed above, including reporting on ancillary testing and any other pertinent documents from the patient’s medical record not mentioned.

  • If patient billing is requested, include patient demographic information, including insurance information. If unable to bill patient, the requesting client will be billed.

  • Securely pack material in a padded packaging and plastic slide holders. If including a paraffin block, especially in warmer months, include an ice or gel pack to ensure that the blocks do not melt in transit.

  • Send materials to the below address using your mail carrier (FedEx is preferred). Select 1-2 day shipping.


Specimen Requirement: Formalin-fixed paraffin-embedded tissue


Special Instructions: Collect all diagnostic slide preparations, such as hematoxylin and eosin (H&E), special stains, immunohistochemical slides and include any relevant pathology report(s), a complete clinical history, and a brief explanatory note. If special studies such as flow cytometry, FISH, IHC, or IF are required, submit additional tissue in the appropriate fixative.

The following major insurance providers are currently accepted by Pathology Laboratory Associates; however, we actively monitor out-of-network claims and evaluate adding additional plans to our accepted insurances list. If you do not see your insurance plan below, please contact us and our billing staff would be more than happy to provide you the information on your insurance’s in-network status with Pathology Laboratory Associates.

LabWorks Login Directions

PLA recommends the use of Google Chrome with LabWorks.

To log on to LabWorks, you will need to disable your browser’s “Pop Up Blocker” feature for the website
https://rmllabworks.rmlonline.com. To do that, in Chrome:

  1. Click on the three dots in the upper right corner of Chrome to the right of the address bar, then click settings.
  2. In the settings menu, select the “Privacy and security” option on the left of the screen,
  3. then select the site settings option in the middle pane.
  4. In the site settings menu, scroll down to the “Pop-ups and redirects” menu and select it.
  5. From the Pop-ups and redirects menu, click add on the “Allowed to send pop-ups and redirects” line.
